Consultations
All consultations are recommended to obtain a referral from your physician. This can be a primary care physician, a specialist physician , or a chiropractic physician. Dr. Wolfe does perform some consultations without a referral, but without prior records, the benefit of the consultation may be rather limited. After the visit, a consult report will be sent to your physician containing diagnoses and recommendations.
Evaluation and Management
All new patients that are expected to require treatment are strongly recommended to obtain a referral from your physician. This can be a primary care physician, a specialist physician or a chiropractic physician. We also accept referrals from psychologists and physical therapists. Dr. Wolfe does accept some new patients without a referral, but will only make a limited evaluation and is unlikely to provide any treatment until he has obtained appropriate records from prior treating health care providers. A report will be sent to the referring physician or health care provider that contains diagnoses and a treatment plan.
The treatment plan will be instituted and may include medications, physical therapy, chiropractic, counseling and injections. Treatment will continue until stopped by the doctor because the patient is better or because all reasonable treatments have been exhausted. The patient may also choose to stop treatment.
Independent Medical Examinations
Certain Ohio Bureau of Workers Compensation patients may be referred by BWC for an independent exam with Dr. Wolfe. An IME report will be sent to BWC.
